Why Pain Management Needs Better Options
Over-the-counter NSAIDs carry gastrointestinal and cardiovascular risks with long-term use. Opioids carry serious addiction potential. For the millions living with chronic pain, hemp-derived cannabinoids offer a safer complementary option backed by a growing evidence base.
How Hemp Targets Pain
Hemp cannabinoids reduce pain through several distinct mechanisms:
TRPV1 desensitisation — CBD binds to vanilloid receptors (the same ones capsaicin targets), reducing their sensitivity to pain signals. This is particularly effective for heat and inflammatory pain.
Endocannabinoid modulation — By preventing anandamide breakdown, CBD reduces pain signalling in the spinal cord and brain's pain processing centres.
Inflammation reduction — CBD inhibits prostaglandin synthesis and cytokine release — the same basic mechanism as NSAIDs, but through a different pathway.
Central sensitisation — In chronic pain conditions, the nervous system becomes overactivated (central sensitisation). CBD and CBG may help "reset" this hypersensitivity.
Pain Types and Hemp Approaches
| Pain Type | Best Product | Why |
|---|---|---|
| Arthritis joint pain | Topical + oral CBD | Topical for local relief, oral for systemic inflammation |
| Neuropathic pain | Full-spectrum oil | Entourage effect; higher doses needed |
| Muscle soreness | CBD topical balm | Fast local absorption |
| Headache / migraine | Sublingual oil | Fast onset; CBD may reduce migraine frequency |
| Fibromyalgia | High-dose oral CBD | Addresses central sensitisation |
| Post-surgical pain | Oral CBD (adjunct) | Discuss with surgeon; may reduce opioid requirement |
Key Studies
A 2020 systematic review (European Journal of Pain) of 18 randomised controlled trials found that CBD reduced pain scores significantly more than placebo across multiple pain conditions.
A 2016 rat study showed topical CBD gel significantly reduced joint swelling and pain behaviour in arthritis models, without systemic side effects.
A 2018 survey of 2,409 hemp users found pain was the most common reason for use (64%), with 80% reporting that hemp was effective or very effective.
Getting Started
- Start with a mid-potency CBD oil (20–25mg per serving)
- Take consistently for at least 2 weeks before assessing
- Add a topical for any localised pain on top of oral dosing
- Track your pain scores daily to measure progress
- Adjust dose upward by 5–10mg increments if needed
